The Center for Health and Wellness has policies and protocols in place to ensure your protection in matters of health, privacy, and fair treatment while attending Capital University. The policies below address medical excuses, confidentiality, and our ADD/ADHD assessment practices.
ADD/ADHD Protocol
- Students may be referred for testing from professors, counselors, and/or may self-refer.
- All new evaluations will include a physical examination by a physician and refer to The Ohio State University Medical Center for formal ADD/ADHD testing.
- Depending upon testing results, the physician may prescribe ADD/ADHD medications.
- Follow up regarding diagnosis and medication will be at the discretion of the prescribing physician.
- If previously diagnosed, student will provide the CHW with copies of formal ADD/ADHD testing and physician may prescribe medications otherwise testing will need to be repeated.
Policy on confidentiality
Students age 18 and older have a legal right to receive confidential services at the Center for Health and Wellness. Therefore, students must sign a Release of Confidential Information form if they would like anybody outside of the CHW (including their parents) to receive information about their health care (with exception for a health emergency). We comply with the Family Education Rights & Privacy Act (FERPA).
Students age 17 and younger cannot receive medical or counseling services unless their parent/legal guardian signs a Consent to Treatment form.

Medical excuse policy
The Capital University Center for Health and Wellness does not issue medical excuses for missed classes. Students are responsible for notifying professors of their absence from class due to short term illness/injury. However, if the student signs a Release of Information form after their ill visit, professors may call the Center for Health and Wellness to verify attendance of the visit only and the reason for absence will not be disclosed.
THE STUDENT IS RESPONSIBLE FOR CONTACTING EACH PROFESSOR TO DISCUSS MISSED CLASSWORK.
